I would appreciate information about a module and smart card for a 9200T - where to buy them, costs, advice about fitting, and firm information about just what channels would be available.

AFAIK (Glossary of Common Abbreviations and Acronyms) only ESPN is available now through the CAM/card. CAMs and cards should be available at TV or satellite shops eg http://www.turbosat.com/categories/CAM%27s/ESPN-Cam-/
A google of "ESPN CAM" should bring up more suppliers.Top-up TV used to be available using a CAM/card but they now require their own hardware to be used.
Fitting is easy - the card slides into the CAM and the CAM just needs to pushed into the slot fully - that's it!
